Type 2 Diabetes
We see people who meet the follow criteria:
DIABETES - Type 2
- Patients with persistent raised fasting glucose > 7.0 mmol/l or HbAIc7.5% or 58 mmol/mol
- Who have not responded to initial first line advice
- Patients with Type 2 diabetes with a cholesterol above 4mmol/l or LDL > 2mmol/l
- Patients who have poorly controlled diabetes who also have complications such as renal impairment, leg ulcers, CVD and/or hypoglycaemia.
Impaired Glucose Tolerance or Impaired Fasting Glucose
- Patients with IGT or IFG who have not responded to first line advice

How to get referred
If you are registered with a GP in Haringey, your GP or healthcare professional must refer you. If you are registered with a GP in Islington, you can self-refer using the online form below or your GP can also refer you.
