Supported Discharge and Rehabilitation Team
The Supported Discharge and Rehabilitation Team (SDRT) aim to provide a seamless discharge service from hospital to home with subsequent rehabilitation input of up to 4 weeks.
The team has a large role in providing early supported discharge and preventing re-admission into hospital. When a client is identified as being appropriate for the team (following screening on the ward), an access visit is done to assess the home enviroment and to see if any equipment is needed. Following this, a date for discharge is set with the ward and the client in then taken home in the SDRT or hospital transport. The client then begins their period of rehabilitation in their own home.
